Parent Permission Form
On Target Indoor Range requires the Parental Consent Form found above and below. Any adult bringing a minor to the range must have the express written permission of the parent or other legal. This consent form is mandatory for every minor at the range not accompanied by their parent or legal guardian.

CALIFORNIA PENAL CODE SECTION 12552:
S 12552: Furnishing Firearms to Minors under 18 without permission of parent. — Every person who furnishes any firearm, air gun, or gas-operated gun, designed to fire a bullet, pellet, or metal projectile, to any minor under the age of 18 years, without the express or implied permission of the parent or legal guardian of the minor, is guilty of a misdemeanor. All employees are NRA Certified Range Safety Officers (RSO’s) to ensure your safety while at the range. We have a Chief Range Safety Officer on staff to help in our continued education in Range Operations. Nearly all of the RSO’s have additional NRA Instructor Certifications in: Pistol, Rifle, Shotgun, Personal Protection Inside the Home, Personal Protection Outside the Home, and several are registered Boy Scout Rifle Merit Badge Counselors in the Orange County Council.
Important Announcement Regarding Compliance
with California’s Proposition 65
Dear Valued Customer,
California’s “Proposition 65” law requires companies conducting business in California to provide a “clear and reasonable” warning before knowingly and intentionally exposing anyone to the specified chemicals known to the State of California to cause cancer and birth defects or other reproductive harm.
Effective August 30, 2018, changes to Proposition 65 will require changes to the product specific warning in addition to new notification warnings to the purchaser as follows:
•For internet purchases, a warning or clearly marked hyperlink must be provided on the product display page or by otherwise prominently displaying the warning to the purchaser prior to completing the purchase.
•For catalog purchases, a warning must be provided in the catalog in a manner that clearly associates it with the item being purchased.
An example of a Proposition 65 warning for firearms and ammunition is as follows:
WARNING This product can expose you to chemicals including lead, which is known to the State of California to cause cancer and birth
defects or other reproductive harm. For more information go to www.P65Warnings.ca.gov.
In order to assist our customers with compliance, RSR Group will identify the products we offer that are subject to Proposition 65 based on notifications we receive from manufacturers. In addition, we will provide a Proposition 65 indicator as well as the warning message in our download files which can be found online in the “Dealer’s Toolbox” under “Available Downloads”. A complete list of products subject to Proposition 65 will be available on our website in the Online Catalog under Prop 65 (www.rsrgroup.com/Prop65) as of August 30, 2018. This will be updated regularly as we receive additional information from manufacturers.
